FAI into the death of Robert Shearer Campbell

Description

This was an FAI into the death of Mr Campbell who died following an accident in the course of his employment with ASCO, when he fell from the steps of a forklift truck landing on his coccyx, falling backwards and striking the back of his head on the ground. He declined to go to hospital but later that morning, he went to hospital. He was diagnosed with a significant brain injury. He received surgery to his brain. Following that surgery Mr Campbell did not regain consciousness and it became clear that he would not recover and life support was terminated. The court considered if the accident and subsequent death could have been avoided.
